New Student Registration Procedure
Box Elder Middle School's New Student Registration Procedure
By Appointment Only
A parent or guardian needs to be present at time of registration and is responsible for providing Box Elder Middle School with the following documents before the enrollment process can begin.
Birth Certificate, Immunizations, Withdrawal Form with Grades, Past Report Card or Transcript
A hard copy of the above documents can be presented in person or faxed to:

The following documents will need to be filled out during enrollment. The forms on colored paper will need to be filled out in the Counseling Office but they can be filled out at home and then you can transfer the information once you get into our offices.

Home Language Survey
Without the above documents we are unable to enroll your student. If your student has not been officially withdrawn from the previous school, you will need to do so bfore the enrollment process here at Box Elder Middle School can be done.
After all documents are in hand, an appointment can be made with Counseling Secretary at (435) 734-4884.
